Advocate Aurora Health is seeking a Registered Nurse (RN) Hospice for a nursing job in Chicago, Illinois.

30pm WITH EVERY 3RD weekend and 1 summer and 1 winter holiday required.
This position is also eligible for up to a $10000.
00 sign on bonus and subject to all taxes! You must be employed in the exact position at time of payout.
Internal team members are not eligible for this sign on bonus at this time.
JOB and UNIT
Summary:
The admission nurse assumes primary responsibility for the admission of patients into the hospice program.
The admission process includes the assessment planning and implementation of the plan of treatment.
The admission nurse ensures admission to the hospice program and smooth transition and hand off to the IDT.
Communication with the hospital physicians and other care providers is essential.
Responsible for the admission process to hospice services.

Demonstrates a commitment to meeting the learning needs of patients and families.
Utilizes appropriate resources to meet those needs and achieve positive patient outcomes.

REQUIRED
Qualifications:
Graduate from an accredited nursing program2-3 years med/surg nursing experience.
Hospice experience recommended Strong computer skills Ability to travelCurrent CPRCOMPENSATION:
