Role Description This is a full-time remote role for a Hospice RN Case Manager at Entera Hospice.
The RN Case Manager will be responsible for providing compassionate and skilled nursing care to patients in the Wester Suburbs/Chicago area.
The role will involve collaborating with a multidisciplinary team to develop and implement individualized care plans, providing direct patient care, coordinating hospice services, and conducting ongoing assessments.
Qualifications Medicine, Palliative Care, and Nursing skills Experience in home care and direct patient care Excellent assessment and critical thinking skills Strong communication and interpersonal skills Ability to work independently and remotely Current RN license Certification in hospice and palliative care is a plus Experience in end-of-life care is preferred Bachelor's degree in Nursing or related field Recommended Skills Assessments Communication Critical Thinking Health Care Home Care Hospice Apply to this job.
